Combined colon + rectum scope. The two share most systemic-therapy decisions (FOLFOX/CAPOX adjuvant, FOLFIRINOX/FOLFOX+bev/cetux/panit metastatic, pembro MSI-H 1L) but rectum has additional neoadjuvant CRT / TNT decisions that the current schema cannot cleanly express (see PROPOSAL §17). For now, rectum-specific neoadjuvant logic is documented in Indication.notes free-text; full sequencing awaits proposal ratification. Required biomarkers per NCCN colon v4.2025 §pretreatment for any stage III–IV: MSI / dMMR (treatment-defining for MSI-H), RAS extended (KRAS exons 2-4 + NRAS exons 2-4), BRAF V600E, HER2 (rare but actionable in RAS-WT).
