| Typische Dosierung | CAR-T-Lymphodepletion: 30 mg/m2 IV täglich × 3-4 Tage. CLL FCR: 25 mg/m2 IV Tage 1-3 alle 28 Tage × 6 Zyklen. Salvage-Lymphom (FluMel-Konditionierung): 30 mg/m2 IV Tage 1-5. Anpassung der Nierendosis: CrCl 30-70 ml/min reduzieren 25%; CrCl < 30 ml/min erwägen 50% zu vermeiden oder zu reduzieren. |

Warnungen
- Schwere und verlängerte Myelosuppression
- Schwere Immunsuppression (CD4-Lymphopenie + opportunistische Infektion — PJP, CMV, HSV-Reaktivierung)
- Transfusion-associated GVHD — irradiate ALL blood products lifelong post-fludarabine
- Autoimmunhämolytische Anämie (insbesondere in CLL)
- Neurotoxizität bei hohen Dosen (Enzephalopathie, kortikale Blindheit)
Anmerkungen
Backbone of CAR-T lymphodepleting conditioning (Flu/Cy). Also historic CLL backbone (FCR), now largely supplanted by BTKi / BCL2i. CRITICAL: irradiate ALL blood products LIFELONG after fludarabine exposure to prevent transfusion-associated GVHD. PJP + viral prophylaxis mandatory during + ≥6 months after treatment. Renal dose adjustment essential. Ukraine: registered + НСЗУ-reimbursed.
