56F postmenopausal × 4 yrs presents with multi-system concerning findings discovered during workup of unrelated symptoms: (1) Workup of microscopic hematuria revealed 4.0 cm contrast-enhancing exophytic kidney_mass right upper pole on CT (RENAL nephrometry 8); (2) Routine dermatology screening identified atypical pigmented growth on dorsal foot 1.1 cm with ABCDE positive (asymmetry + irregular border + variegated color + diameter + recent enlargement) — melanoma_suspect; (3) TVUS during postmenopausal cervical screening showed endometrial_thickening 12 mm without clinical symptoms (no PMB) — endometrial workup also indicated. PCP appropriately concerned for synchronous multi-primary cancers vs metastatic disease vs hereditary cancer syndrome. No family history of specific cancers but limited family knowledge. Workup needs coordinated biopsy planning across 3 sites + germline panel for multi-cancer susceptibility (BAP1, BRCA1/2, HRR, Lynch).
