Red Flag Origin

DefinitionIn-utero exposure to diethylstilbestrol (DES) — synthetic non-steroidal estrogen prescribed 1940-1971 (USA) and 1948-1978 (Europe) to women with prior miscarriage or threatened miscarriage. DES-exposed daughters carry ~40-fold increased risk of clear-cell adenocarcinoma of the vagina and cervix (peak incidence ages 15-30 but cases continue into older ages), ~2-fold lifetime risk of breast cancer (post-menopausal incidence doubles after age ~40), and structural reproductive anomalies (T-shaped uterus, vaginal adenosis, cervical hood). DES-exposed mothers themselves have ~30% increased breast cancer risk. Although the exposed cohort is aging out (most exposures pre-1978), surveillance continues lifelong. Prevention-persona RedFlag (§20 v0.3 reproductive expansion). Cohort still in active surveillance pathway through ~2050 given peak prescription years.

Prevention-persona RedFlag for DES in-utero exposure cohort. Surveillance is intensified relative to general population (annual pelvic with careful vaginal/cervical inspection; colposcopy if abnormal cytology; mammography at younger age threshold per NCCN modifier risk; consider MRI breast screening adjunct given doubled risk). STUB pending two-Co-Lead signoff per CHARTER §6.1 dev-mode.
