DefinitionChronic HCV infection (HCV-RNA detectable / persistent viremia) in an individual without a current B-cell NHL diagnosis. IARC Group 1 carcinogen (HCV) — established etiological driver of indolent B-cell lymphomas (MZL, lymphoplasmacytic, splenic-MZL, low-grade DLBCL subset) and HCC. DAA-based curative antiviral therapy is the highest-leverage prevention pathway and reduces both NHL incidence and HCC incidence post-SVR12. Prevention-persona RedFlag (§20 ratification 2026-05-18).

Notes

Prevention-persona RedFlag (CHARTER §3 amended 2026-05-18 Path A, HCP-mediated). Fires when patient profile shows chronic HCV (HCV-RNA detectable OR HCV-Ab+ with persistent viremia >6 months) AND no confirmed B-cell NHL diagnosis. Engine routes such patients to a PreventionPlan recommending DAA antiviral therapy (per IND-CHRONIC-HCV-PREVENTION-DAA) as the etiological-cancer-prevention pathway. Evidence for HCV → NHL causation (IARC Group 1): multiple cohort studies show 2-3x increased risk of indolent B-cell NHL in chronic HCV carriers; epidemiology especially robust for MZL subtypes. DAA cure (SVR12) has been associated with reduced NHL incidence in post-treatment cohorts (de-Re et al. and others) and with regression of established HCV-associated indolent NHL (the basis of IND-HCV-MZL-1L-ANTIVIRAL for the treatment persona). This is a STUB pending two-Clinical-Co-Lead signoff per CHARTER §6.1 dev-mode exemption (sources cited, draft flag set). Engine consumption is DEFERRED to v0.2 implementation wave — the PreventionPlan code path does not yet exist; this RF lands as schema-valid declarative content awaiting that wave.
