Drug Facts

ClassNorepinephrine-dopamine reuptake inhibitor (NDRI); also nicotinic antagonist
MechanismWeak inhibitor of norepinephrine and dopamine reuptake; non-competitive antagonist at nicotinic acetylcholine receptors. Mechanism in tobacco cessation: dopaminergic effects partially substitute for nicotine's reward signal; nicotinic antagonism reduces reinforcement. Second-line monotherapy for tobacco cessation (USPSTF; lower efficacy than varenicline but similar to NRT).
Typical dosingPO: 150 mg once daily × days 1-3, then 150 mg BID (start ≥1 week before target quit date). Total course 7-12 weeks; extend to 6 months if successful at week 12. Use SR formulation specifically (IR has higher seizure risk; XL is alternative once-daily option). Renal/hepatic impairment: reduce frequency to once daily or every 2 days.

Notes

STUB — v0.2 prevention-workstream authoring; pending two-Clinical-Co-Lead signoff per CHARTER §6.1 dev-mode. Bupropion SR (Zyban indication) is USPSTF/AHCPR-recommended for tobacco cessation; second-line after varenicline due to lower efficacy and seizure-risk contraindications. Particularly useful when comorbid depression or weight-gain concerns. AVOID in patients also taking tamoxifen (CYP2D6 inhibition reduces active metabolite endoxifen). Source cited (SRC-NCCN-BCELL-2025) is closest in-KB until USPSTF/AHCPR sources land in source-stub workstream.
