ANCHOR (2022) demonstrated HSIL treatment reduces anal cancer incidence in HIV+ adults ≥35 — operationalises anal cytology screening for HIV+, MSM, immunosuppressed (solid-organ transplant), women with HPV-driven cervical/vulvar HSIL. Abnormal cytology → high-resolution anoscopy (HRA) + targeted biopsy. Universal screening not USPSTF- endorsed; targeted high-risk screening per IANS / HIVMA / NCCN.
