Notes

Well-differentiated pNET G1/G2 is the dominant subtype in clinical practice (non-functional, discovered incidentally or via abdominal mass/liver mets). Grading per WHO 2019: G1 Ki67 <3%, G2 Ki67 3–20%. G3 well-differentiated pNET (Ki67 >20%) occupies a grey zone — still well-differentiated morphologically but higher proliferation; NCCN recommends everolimus/sunitinib/SSA ± chemo; not modelled separately here (flagged via RF-NET-HIGH-GRADE-G3 for MDT review). Distinguishing from poorly differentiated NEC (ICD-O 8246/3) is essential: NEC requires platinum-etoposide chemotherapy (not SSA/TKI/mTOR). Key trials: RADIANT-3 (everolimus, Yao NEJM 2011), Raymond 2011 (sunitinib), CLARINET pNET subgroup (lanreotide). Surgery remains the only curative option for localized disease.
