Notes

GI-NET (non-pancreatic) is the most common NET overall. Midgut/ileal carcinoids are prototypical: indolent growth, somatostatin receptor expression (SSTR2+), liver/mesenteric metastases, carcinoid syndrome (serotonin-driven flushing + diarrhea). Grading per WHO 2019: G1 Ki67 <3%, G2 Ki67 3–20%. Carcinoid syndrome occurs when serotonin (and other mediators) bypass hepatic metabolism — typically with liver metastases or retroperitoneal disease. SSA (octreotide/lanreotide) are the foundation of treatment: (1) symptom control and (2) antiproliferative (CLARINET, PROMID). PRRT (¹⁷⁷Lu-DOTATATE; NETTER-1 trial, Strosberg NEJM 2017) is approved for SSTR2+ midgut NETs after SSA progression — pending KB SSTR biomarker scaffolding (deferred). Key trials: CLARINET (lanreotide, Caplin NEJM 2014), PROMID (octreotide LAR, Rinke JCO 2009), NETTER-1 (PRRT, Strosberg NEJM 2017).
