  • Do NOT prescribe HD-MTX at CrCl <50 mL/min OR third-space fluid — substitute reduced MTX with intensified rescue.
  • Do NOT skip aggressive IV hydration + urine alkalinization (pH ≥7.5) before HD-MTX.
  • Do NOT forget leucovorin rescue 24h post-MTX.
  • Do NOT ignore delayed MTX clearance — glucarpidase rescue when MTX rising >100 µmol/L at 24h.
  • Do NOT prescribe WBRT in patients >60 years without discussing cognitive toxicity risk.
  • Do NOT forget PJP prophylaxis (cotrimoxazole) ≥6 months after last anti-CD20 dose.
