58M presents with 6-month history of aquagenic pruritus (intense itching after warm shower), facial plethora, occasional headache, blurred vision, and one episode of digital ischemia (transient erythromelalgia) of right index finger. CBC over 3 visits: Hgb 19.2 g/dL (markedly elevated, baseline 14.8 two years prior), Hct 58%, WBC 13.4 (leukocytosis), platelets 562 (mild thrombocytosis), erythrocytosis confirmed. Splenomegaly 16 cm on US. Serum erythropoietin LOW 2.1 mU/mL (suppressed — appropriate for primary erythrocytosis vs elevated in secondary). LDH mildly elevated 245. No tobacco / no chronic hypoxic disease / no testosterone supplementation / no high-altitude exposure. JAK2 V617F testing pending. Strong suspicion of polycythemia vera per WHO 2022 major criteria 1 (Hgb >16.5 M / >16 F) + minor low EPO.
