71M asymptomatic, comes for routine annual physical. PCP labs revealed serum_globulin_high 4.8 g/dL (total protein 8.4, albumin 3.6 — elevated globulin gap). Reflex SPEP confirmed monoclonal_protein 8 g/L IgG-kappa (m_spike_incidental finding). No CRAB criteria — Hgb 13.2, Cr 0.9 (CrCl 78), Ca 9.4, no skeletal symptoms, no lytic lesions on screening skeletal survey. No fatigue, no recurrent infections, no peripheral neuropathy, no hyperviscosity symptoms. Serum free light chains: kappa 24, lambda 18, ratio 1.33 (NORMAL <1.65). β2-microglobulin 2.1 (normal). Per IMWG 2014 criteria — likely low-risk MGUS (size <1.5 g/L, IgG type, normal FLC ratio = ZERO risk factors). Needs IMWG risk stratification + paraprotein_screen + BM biopsy decision (skip if low-risk + normal FLC ratio per current 2014 guidelines).
