Походження тривожної ознаки

Визначення	Current або prior tamoxifen exposure ≥5 років у жінки з intact uterus (без prior hysterectomy). Tamoxifen — selective estrogen-receptor modulator (SERM) — antagonist у breast tissue (therapeutic для ER+ breast cancer + risk reduction) але partial agonist в endometrium → ~2-7 fold увеличений endometrial cancer risk (переважно endometrioid; менш часто serous, carcinosarcoma). Cumulative risk ~1-2% за 5р treatment; вищий при longer duration + post-menopausal status. Endometrial cancers у tamoxifen-exposed жінок часто more aggressive histology порівняно з spontaneous. Population з hysterectomy: цей RF НЕ fires (no at-risk organ).

Prevention-persona RedFlag for tamoxifen exposure ≥5y in women with intact uterus. ~2-7 fold endometrial cancer risk. Engine routes to PreventionPlan recommending symptom-prompted evaluation (postmenopausal bleeding → immediate workup with TVUS + endometrial biopsy). Routine asymptomatic TVUS screening NOT recommended per ACOG (false-positives from tamoxifen-related subepithelial changes). STUB pending two-Clinical-Co-Lead signoff per CHARTER §6.1 dev-mode.
