Походження тривожної ознаки

ВизначенняТривала експозиція thiopurines ≥3-5 років — azathioprine або активного метаболіту 6-mercaptopurine (6-MP) — у IBD (хвороба Крона, виразковий коліт), ревматоїдному артриті, СЧВ, autoimmune hepatitis, transplant maintenance, інших autoimmune показах. IARC Group 1 канцероген (azathioprine). Mechanism: thiopurine метаболіти incorporate у DNA → impaired mismatch- repair + photo-sensitization (UV-A absorption за 6-thioguanine у DNA generates ROS). Risk profile: лімфома 4-6x population risk (переважно DLBCL); non-melanoma skin cancer (особливо SCC) 4-10x background, magnitude scaling з cumulative dose-years і UV exposure; модерний melanoma signal; у IBD specifically: EBV-driven post-thiopurine lymphoma у EBV-seronegative young men (avoid azathioprine у EBV-seronegative young-adult IBD per ECCO 2019).
