| Механізм дії | Cytosine analog phosphorylated intracellularly to lamivudine triphosphate, which competes with dCTP for incorporation into HBV DNA / HIV RNA — leading to chain termination. Active against both HIV and HBV. For HBV: lamivudine has been largely supplanted by entecavir, tenofovir DF, and tenofovir AF due to high rates of resistance (~70% at 5 years). Retains role in resource-limited settings, post-transplant prophylaxis, and as part of HIV ART. |

Застереження
- Lactic acidosis and severe hepatomegaly with steatosis (class effect of NRTIs)
- Post-treatment exacerbations of hepatitis B if discontinued — monitor LFTs and HBV-DNA closely after stopping
- Different formulations for HIV vs HBV — risk of HIV resistance if HBV-strength lamivudine used as HIV monotherapy in unrecognized HIV-HBV coinfection (always screen HIV before HBV monotherapy)
Нотатки
Ламівудин — старіший препарат для HBV, значно витіснений ентекавіром і тенофовіром AF/DF через високу частоту резистентності (~70% за 5 років монотерапії). Зберігає роль у: ВІЛ-ART (у поєднанні з іншими агентами), post-transplant HBV-профілактиці (з HBIg), у країнах з обмеженими ресурсами. НЕ використовувати як монотерапію при ВІЛ.
