Granin-family secretory protein, ubiquitously expressed in neuroendocrine cells. Used as a non-specific biomarker for GEP-NET (gastroenteropancreatic neuroendocrine tumors), MEN1-associated NETs (pancreatic, foregut), and pheochromocytoma (but plasma metanephrines preferred for pheo). NCCN recommends CgA as ADJUNCT, not standalone — annual surveillance in MEN1 carriers (pancreatic NET screening) typically combined with cross-sectional imaging (MRI or EUS) and gastrin/insulin/glucagon/VIP/PP panels per symptoms. Major confounders driving false-positive elevations: proton-pump inhibitor use (most common cause of elevation in clinical practice), atrophic gastritis, renal impairment, heart failure, chronic hepatitis, IBD. Less sensitive than gut-hormone panel for functional NETs. Limited utility for non-functioning small NETs. STUB pending two-Co-Lead signoff.
