DefinitionVisceral organ involvement (pulmonary, gastrointestinal, or other visceral site) OR extensive/rapidly progressive mucocutaneous disease (widespread cutaneous lesions, tumor-associated lymphedema, or ulceration) in Kaposi sarcoma of any epidemiologic form (AIDS- associated, iatrogenic, classic, or endemic). Symptomatic/extensive disease of this kind is not expected to be adequately controlled by immune restoration (ART / immunosuppression rebalance) or local therapy alone and drives escalation to systemic chemotherapy.

Distinct from the pre-existing PREVENTION-persona RF-CHRONIC-HHV8-MALIGNANCY-PREVENTION, which fires on HHV-8 seropositivity in an immunocompromised host BEFORE any Kaposi diagnosis. New finding keys (kaposi_visceral_involvement / kaposi_pulmonary_involvement / kaposi_gi_involvement / kaposi_extensive_cutaneous_disease) -- no equivalent finding existed elsewhere in the KB. INDEPENDENT REVIEW CORRECTION (2026-07-11): an earlier draft of this trigger also fired on a "kaposi_actg_tumor_stage: T1" finding, invoking the ACTG/Krown Kaposi-sarcoma tumor-extent staging system.
