Drug Facts

ClassSynthetic prostaglandin E1 analog
MechanismSynthetic prostaglandin E1 analog with gastric mucosal protective effects (mucus + bicarbonate secretion, decreased gastric acid via enterochromaffin-like cell inhibition) and uterotonic activity. In oncology / cancer-prevention context: investigated as endometrial protection in tamoxifen users (theoretical rationale — endometrial PGE pathways). Evidence base is limited and inconsistent; not guideline-recommended for routine tamoxifen-associated endometrial protection. Primary FDA indications are NSAID-induced peptic ulcer prophylaxis and (off-label) obstetric uses.
Typical dosingNSAID ulcer prevention: 200 mcg PO QID with food and at bedtime (alternative: 200 mcg BID-TID for tolerability). Tamoxifen endometrial protection (off-label, research context only): 200 mcg PO regimens have been studied — NOT a guideline-supported use; clinician judgment + IRB if pursued.

Notes

STUB — v0.2 prevention-workstream authoring; pending two-Clinical-Co-Lead signoff per CHARTER §6.1 dev-mode. PRIMARY indication is NSAID-induced peptic ulcer prevention. CONTROVERSIAL alternative-only use for tamoxifen-associated endometrial protection — NOT NCCN/ESMO recommended; small studies have not shown clear benefit. Engine should NOT auto- recommend; reserved as clinician-judgment alternative pending dedicated guideline. Strict pregnancy exclusion mandatory. Source cited (SRC-NCCN-BCELL-2025) is closest in-KB until dedicated peptic-ulcer- prevention or tamoxifen-endometrium sources land.
