Drug Facts
| Class | Selective estrogen receptor degrader (SERD) — research-only chemoprevention context |
|---|---|
| Mechanism | Pure estrogen-receptor antagonist that binds ER-α and induces its proteasomal degradation — no agonist activity in any tissue (true ER downregulator). Standard of care SECOND-LINE ENDOCRINE TREATMENT for HR+/HER2-negative metastatic breast cancer, especially after AI failure (paired with CDK4/6i, alpelisib, capivasertib). The chemoprev- ention RATIONALE is purely mechanistic — total ER blockade should theoretically prevent ER-driven carcinogenesis — but there is NO RCT-level primary-prevention evidence and the IM injection route plus toxicity profile preclude use in disease-free women. Listed here as a class-completeness entity for the SERD class only; engine MUST NOT surface for prevention. |
| Typical dosing | TREATMENT (metastatic HR+ breast cancer — for context): 500 mg IM on days 1, 15, 29, then 500 mg IM monthly. Two 250 mg / 5 mL injections, one in each gluteal muscle. CHEMOPREVENTION: NO established dose. NOT recommended for chemoprevention in any guideline. Engine should NEVER surface this drug for primary prevention. |

Notes
STUB — v0.2 chemoprevention-workstream authoring (batch 3); pending two- Clinical-Co-Lead signoff per CHARTER §6.1 dev-mode. CLASS-COMPLETENESS entity for the SERD class — distinct from DRUG-FULVESTRANT (the treatment-context entity at drugs/fulvestrant.yaml). NO primary- prevention RCT exists for fulvestrant; the IM route + monthly clinic visit burden + toxicity profile preclude use in disease-free women. USPSTF 2024 (Grade B chemoprevention recommendation) and NCCN-Breast 2025 list tamoxifen, raloxifene, exemestane, anastrozole as chemoprevention options — NOT fulvestrant. Engine MUST NOT surface fulvestrant as a chemoprevention option. This entity exists only so SERD-class queries return a member; it is a research-only / discussion entity. Newer oral SERDs (elacestrant, giredestrant, camizestrant, imlunestrant) are in trials for treatment indications only and are also NOT prevention candidates as of 2026-05.
