Capabilities

Notes

STUB — pending Clinical Co-Lead approval per CHARTER §6.1. CT chest з biopsy of mediastinal mass — initial workup. Avoid steroids перед biopsy if possible. CNS prophylaxis not routine. Differential — anterior/superior mediastinal mass: sarcoidosis (D86.0–D86.2) can present identically to PMBCL on CT (bulky mediastinal mass, bilateral hilar nodes, pleural/pericardial effusions). PET/CT uptake indistinguishable. Biopsy essential; sarcoid granulomata (non-caseating, CD15−/CD30−, no malignant large B-cells) exclude PMBCL. ACE, serum calcium, BAL cell differential help pre-biopsy. Do not start R-CHOP / DA-EPOCH-R on imaging alone.
