Quantitative HBV DNA used to TRACK viral suppression over time during chronic-HBV antiviral therapy (entecavir or tenofovir TDF/TAF — first- line nucleos(t)ide analogues per AASLD 2024 / EASL 2025). Distinct from BIO-HBV-STATUS (serology-based screening / treatment-decision gate). Two clinical contexts: (1) Treatment monitoring in HBeAg+/HBeAg− chronic HBV: baseline HBV DNA + ALT + HBeAg + HBsAg → start NA → recheck q3 months for first year, then q6 months. Goal: HBV DNA undetectable on treatment; ALT normalization; HBeAg seroconversion (HBeAg+ patients) or HBsAg loss (functional cure — rare on NA monotherapy, ~1% per year). Virologic breakthrough (≥1-log rise from nadir) prompts adherence review then switch (entecavir → tenofovir if NA-naïve resistance). (2) Prophylaxis monitoring during B-cell-depleting therapy (rituximab, obinutuzumab) or anti-CD38 (daratumumab, isatuximab) in HBsAg+ or anti-HBc+ patients: entecavir prophylaxis baseline → HBV DNA q3 months during chemotherapy and for 12+ months post-treatment; any detectable HBV DNA = reactivation, hepatology consult.
