Drives surveillance interval and intervention: stage I-II every 2-5 y; stage III every 6-12 mo + endoscopic ablation; stage IV → referral for prophylactic pancreas-preserving duodenectomy (lifetime duodenal cancer risk ~5-10% overall, ~36% at stage IV). Updated 2017 Mayo classification adds ampullary criteria — captured in notes per authoring locality.
