Companion entry to BIO-CTDNA-MRD focused on POST-TREATMENT serial surveillance (vs. single landmark timepoint). Distinct from MCED (BIO-CFDNA-METHYLATION-MCED) in that ctDNA-MRD assumes a known prior cancer diagnosis with post-curative-intent surgery / chemoradiation. Strongest evidence in CRC (GALAXY / CIRCULATE-Japan, Kotani Nat Med 2023): MRD-positive post-op patients ~22% 18-mo DFS vs ~93% MRD- negative; MRD-positive benefit from adjuvant chemotherapy. Emerging evidence in muscle-invasive bladder cancer (IMvigor010 ctDNA subgroup), early breast cancer (c-TRAK-TN), NSCLC (post-IMpower010 retrospectives), pancreatic, gastric. Trial frameworks for MRD-driven escalation/de-escalation: CIRCULATE-Japan ALTAIR/VEGA, NRG-GI008 (CIRCULATE-US), DYNAMIC-IV. ESMO 2024 endorses ctDNA-MRD as a prognostic biomarker for CRC stage II adjuvant decision-making (May inform adjuvant chemo discussion when MRD+ at 4-8 weeks post-op).
