Serum tumor marker (CEACAM5). NOT recommended for general-population CRC screening (low sensitivity for early-stage disease, false positives from smoking, IBD, cirrhosis, pancreatitis). PRIMARY surveillance use: after curative-intent resection of stage II/III CRC — q3-6 months CEA for 2 years then q6 months for 3-5 years (NCCN/ESMO concordant), rising trend triggers imaging workup for recurrence. Two consecutive rises >5 ng/mL or doubling within 1-2 months warrant CT chest/abdomen/pelvis and consider colonoscopy. Approximately 30% of CRCs do not secrete CEA (microsatellite-unstable, mucinous, signet-ring histologies); in those patients CEA surveillance has limited yield. Distinct from BIO-CTDNA-MRD which offers higher analytic sensitivity for molecular residual disease. STUB pending two-Co-Lead signoff.
