Serum tumor marker (sialyl Lewis-a). NOT a screening test for the general population (low PPV — elevated in cholangitis, benign biliary obstruction, cirrhosis, pancreatitis, smoking). CAPS Consortium surveillance protocol for high-risk individuals (CAPS-eligible: BRCA1/2 + family history, PALB2 + family history, ATM + family history, CDKN2A carriers, Peutz-Jeghers, Lynch with FDR PDAC, FAMMM, HNPCC) uses CA 19-9 as an adjunct to annual MRI/MRCP or EUS — not as standalone screening. Also used in cholangiocarcinoma surveillance in PSC (rising trend prompts MRI/MRCP). Useful for monitoring known PDAC during treatment (rising trend correlates with progression in Lewis-positive patients). STUB pending two-Co-Lead signoff.
