MCL has dismal prognosis (median OS <1 year historically). Per NCCN-SM: avapritinib is preferred for MCL (PATHFINDER showed responses in MCL subset); allogeneic HCT considered for fit patients in remission. Direction INTENSIFY → cytoreduction + consider HCT.
