Notes
CALGB 10403 (Stock 2019): 2-3 yr POMP maintenance after induction + consolidation + interim maintenance + delayed intensification. Standard tail of pediatric-inspired BFM-style regimen for AYA Ph-negative B-ALL; also used in adult Ph-negative ALL and post hyper-CVAD adult protocols. Dose-titrated to mild myelosuppression (ANC 0.75-1.5 × 10⁹/L). TPMT/NUDT15 genotyping recommended pre-initiation when available. PJP prophylaxis (TMP/SMX) mandatory throughout maintenance. STUB — requires Clinical Co-Lead signoff.
