Notes

In ECOG 3-4 mCRC patients where full doublet (FOLFOX/CapOx) or triplet (FOLFIRINOX) is not feasible due to performance status, comorbidity burden, or patient preference, single-agent capecitabine offers a low-toxicity oral option that preserves quality of life while providing modest disease control (mPFS ~4-6 mo in unselected populations; longer in MSS / well-differentiated tumours). Avoids IV access requirements and reduces clinic burden. Discuss explicitly with patient + family that the goal is symptom-balance, not life-prolongation in any meaningful sense — this regimen is BSC-adjacent and the boundary between treatment and BSC is fluid. Co-enrol palliative care early. DPYD pre-screening strongly recommended (CPIC) — DPYD deficiency causes severe-fatal toxicity.
