Notes

1L systemic HCC default — Child-Pugh A, ECOG ≤1, varices managed. Etiology rails (parallel to indication selection): HBV+ → entecavir/TDF prophylaxis throughout + ≥12 mo post (RF-HCC-HBV-REACTIVATION-RISK + SUP-HBV-PROPHYLAXIS); HCV-RNA+ → DAA (SOF/VEL or glec/pibr) before or concurrent with atezo+bev. Both pathways operate via parallel supportive_care, not by shifting atezo+bev → durva+treme.
