Notes

REG-PEMBRO-CISPLATIN-5FU-ESOPH (pembrolizumab 200mg q3w + cisplatin 80mg/m² D1 + 5-FU 800mg/m²/d D1-5 q3w) needs to be authored. Carboplatin can substitute for cisplatin in cisplatin-ineligible patients (AUC 5 q3w); less evidence but acceptable in clinical practice. Maximum 35 cycles pembrolizumab (per KEYNOTE design); cisplatin typically given for 6 cycles then 5-FU+pembro maintenance per institutional practice.
