Notes

CAPOX vs FOLFOX choice: patient preference (oral vs IV), renal function, access to infusion centre. Low-risk stage III → CAPOX 3 months is now the preferred standard (less neuropathy, equivalent survival). High-risk stage III → 6 months either regimen, with clinical discussion about neuropathy burden. GI-3 B1 (chunk gi3-b1-idea-stratification-2026-05-08-1900): IDEA-stratified duration recommendation per NCCN 2025 / ESMO 2024 surfaced via duration_guidance (operational) + expected_outcomes stratum fields (DFS HR 0.85 low-risk T1-3 N1 non-inferior; HR 0.79 high-risk T4/N2 favouring 6 mo) + known_controversies (FOLFOX 3-mo failed non-inferiority, constituent-trial heterogeneity). Sister indication ind_crc_adjuvant_stage3_folfox.yaml not edited in this chunk — FOLFOX duration deserves separate consideration (out of B1 scope).
