Capabilities

Notes

Two biological subgroups managed similarly anatomically but with distinct prevention implications: HPV-driven (vaccination prevents precursor lesions; screening with vulvar exam + biopsy of any persistent lesion) vs HPV-independent (lichen sclerosus surveillance is the dominant prevention modality; topical clobetasol may reduce malignant transformation in select cohorts). Treatment: early-stage — wide local excision ± inguinal sentinel node biopsy (GROINSS-V paradigm replacing routine complete inguinofemoral lymphadenectomy); locally advanced — definitive chemoradiation (cisplatin + RT); metastatic — pembrolizumab if PD-L1+ (KEYNOTE-158 cohort). Vaginal SCC managed similarly with RT-based approaches (anatomic constraints limit surgical options).
