CLEOPATRA (Swain 2020, NEJM): pertuzumab + trastuzumab + docetaxel vs trastuzumab + docetaxel — mOS 57.1 vs 40.8 mo. After ~6 cycles of THP induction with confirmed response, docetaxel is dropped and HP maintenance continues until progression. Endocrine therapy added if HR-co-positive. PHESGO (HP fixed-dose SC + hyaluronidase) approved alternative — single SC injection over ~5-8 min, replaces both IV infusions; preferred for chair-time and patient convenience after initial IV doses tolerated. LVEF baseline + q3 mo.
