DefiniciónCLL with acquired BTK C481S (rarely C481R/F/Y) mutation after progression on covalent BTK inhibitor (ibrutinib, acalabrutinib, zanubrutinib). C481S abolishes covalent binding of cBTKi to BTK; non-covalent BTKi (pirtobrutinib) retains activity (BRUIN, Mato Lancet 2021; pirtobrutinib in cBTKi-pretreated CLL ORR 82%, mPFS 19.6 mo). Routes 2L+/3L+ post-cBTKi CLL with confirmed C481-axis resistance to pirtobrutinib over chemoimmunotherapy or alloHCT.

Distinguirse de RF-PRIOR-BTKI-PROGRESSION (universal, amplio) — que señala fuegos en cualquier PD cBTki; esta bandera es la resistencia C481S confirmada por mutación, el mecanismo canónico de la resistencia cBTDKi adquirida (~80% de los pacientes con enfermedad progresiva en cBTTKi). Las mutaciones PLCG2 (R665W, S707F/Y, L845F) son un mecanismo de resistencia alternativo (~15%) — también responde a pirtobrutinib. Prueba en PD por ddPCR o NGS en células mononucleares de sangre periférica (sensibilidad mejor que la BM en CLL). Venetoclax + rituximab (MURANO) y CAR-T (TRANSCEND-CLL-004 con lisocel, ORR 80%, CR 18% en tratamiento previo pesado) son las opciones alternativas para pirtobrutinib-inelegible o progresado pirtbrutinibi; combinaciones en estudio (pirto + ven; pirtto + ven + obinutuzumab — BRUIN-Cll-321/322).
