ATC ist eine der aggressivsten menschlichen malignen Erkrankungen (mediane OS 3-6 Monate historisch). Die Entdeckung, dass ~25-40% BRAF V600E- Mutant transformierte Praxis — Dabrafenib + Trametinib (ROAR Korb) erreicht ORR ~70% mit mediane OS verlängert auf ~14 Mo in BRAF-mutant ATC, manchmal Umwandlung unresectable zu resectable. NCCN Cat 1 Empfehlung; FDA Zulassung 2018. Für BRAF-mut ATC, Neoadjuvant Dabrafenib + Trametinib → Chirurgie → adjuvant RT ist jetzt das optimale Paradigma, wenn machbar. NTRK / RET / ALK Fusionen < 5% kombiniert, aber sehr handlungsfähig. Umfassende genomische Profilierung innerhalb von Tagen der Diagnose ist kritisch gegeben Krankheitstempo. Priorität stieß auf 30, weil gezielte Therapie kritisch-tier Überbrückung der zytotoxischen Standard.
