CHP-Bv (ECHELON-2) for CD30+ PTCL NOS substitutes brentuximab for vincristine — reduces neuropathy vs CHOEP / CHOP. With baseline neuropathy ≥2, even CHP-Bv risky; use GEM-P or pralatrexate or romidepsin monotherapy as palliative-curative balance. LVEF <50%: liposomal doxorubicin substitution; or DHAP-class regimens anthracycline-free for transplant bridge. Severe hepatic dysfunction: brentuximab contraindicated Child-Pugh C; reduce to 1.2 mg/kg Child-Pugh B with monitoring.
