This RF de-escalates from Pola-R-CHP intensification toward R-CHOP / R-mini-CHOP when frailty co-occurs with high-IPI (severity tie-break: hold > intensify > de-escalate, but here directions conflict so engine logs both and surfaces conflict to MDT).
