Notes
ALPINE trial (Brown 2023): in r/r CLL, zanubrutinib superior to ibrutinib in PFS (HR 0.65) and cardiac safety (afib 5.2% vs 13.3%; cardiac death 0% vs 1.9%). SEQUOIA trial: 1L CLL, zanubrutinib vs bendamustine-rituximab — superior PFS. Combined: zanubrutinib is preferred BTKi for both 1L (SEQUOIA) and r/r (ALPINE) CLL where accessible. Acalabrutinib (REG-ACALABRUTINIB-CONTINUOUS, ELEVATE-TN/RR) is the alternative second-generation BTKi with similar safety profile.
