Preferred 1L for advanced/progressive RET-mutant MTC per NCCN (LIBRETTO- 531 randomized vs cabozantinib/vandetanib: superior PFS, fewer treatment-limiting AEs). LIBRETTO-001 ORR ~73% in RET-mutant MTC with median DOR not reached. Patient must have advanced/unresectable disease + symptomatic OR structural progression OR calcitonin doubling time <2 yr. Pralsetinib (ARROW) is the alternative RET-selective.
