Notes
Standard 1L symptom-directed therapy for symptomatic PMF (and post-PV/ET MF). COMFORT-I/II: ≥35% spleen-volume reduction in ~42% vs 0.7% placebo; symptom improvement; pooled OS benefit suggested. Does NOT eliminate clone or alter natural history of fibrosis. AlloHCT remains the only curative option for fit intermediate-2 / high-risk patients. Momelotinib (MOMENTUM) preferred if anemia dominant; fedratinib (FREEDOM-2) 2L; pacritinib for severe thrombocytopenia. NEVER abrupt-stop. STUB — requires clinical co-lead signoff before publication.
