Notes

STUB — pending Co-Lead sign-off. NATALEE: 3-year adjuvant CDK4/6i for HR+/HER2- early breast cancer (stage II-III + selected stage I high-risk). Ribociclib adjuvant dose is 400 mg (NOT the 600 mg metastatic dose). Key distinction from abemaciclib (monarchE): ribociclib has BROADER eligibility (includes node-negative with risk features); however requires mandatory ECG/QTc monitoring at baseline and on-treatment (QT prolongation boxed warning — fatal arrhythmia risk). Concurrent AI required throughout 3 years of ribociclib, then AI continues to 5 years total. Strong CYP3A4 inhibitors increase ribociclib exposure — avoid or dose-reduce. Separate metastatic regimen: REG-AI-RIBOCICLIB (600 mg continuous dose).
