Notes
RELATIVITY-047 phase 2/3: nivo+rela vs nivo mono in 1L melanoma — mPFS 10.1 vs 4.6 mo (HR 0.75). 4-y OS ~48%. Key differentiator vs ipi+nivo: Grade 3-4 treatment-related AE 18.9% vs ~50%. Position in our 2L+ algorithm: IO-naive alternative for patients who received 1L BRAFi+MEKi (no prior anti-PD-1) when ipi-grade irAE risk is unacceptable (e.g., baseline autoimmunity, frailty). Trial was 1L; off-label position requires MDT documentation. Myocarditis surveillance (baseline + serial troponin, ECG with symptoms) recommended.
