Notes
BRUIN MCL cohort (Wang 2023): r/r MCL after prior cBTKi (median 3 prior lines, ~80% prior cBTKi, ~50% prior cBTKi-discontinued for resistance) — ORR 50% (CR 13%); mPFS 7.4 mo; mDOR 17.6 mo. FDA- approved for r/r MCL after ≥2 prior lines including cBTKi. Distinct binding mechanism (non-covalent, C481-independent) overcomes resistance to ibrutinib / acalabrutinib / zanubrutinib. Important bridge to CAR-T (brexu-cel) when manufacturing window concerns exist. Cardiac safety better than ibrutinib (afib ~3% vs ~7-15%).
