KEYNOTE-189 (non-squamous, with pemetrexed maintenance) and KEYNOTE-407 (squamous, no pemetrexed maintenance) established pembrolizumab continuation through 35 cycles total — induction (4 cycles chemo+pembro) followed by maintenance pembrolizumab (± pemetrexed in non-squamous) until progression or completion. 5-year OS ~19-22% vs ~11% chemo alone (KEYNOTE-189 5-year update). Should be given as continuation of original 1L plan — NOT a separate decision point. Q6 weekly 400 mg flat dosing equivalent to 200 mg q3 weeks per population PK; reduces clinic visits.
