Notes

KEYNOTE-671 adjuvant phase (Wakelee NEJM 2023): pembrolizumab 200 mg q3w × 13 cycles (~9 months) starting within 4-12 weeks of complete surgical resection, following 4 cycles of neoadjuvant pembrolizumab + cisplatin-based doublet (REG-PEMBRO-CHEMO-NEOADJ-NSCLC). Total perioperative pembrolizumab exposure: 17 cycles / ~1 year. EFS HR 0.58 and OS HR 0.72 reflect the contribution of the full perioperative package; adjuvant phase contribution is not independently dissectable from the trial design. Distinct from REG-PEMBRO-ADJUVANT-MELANOMA (18 cycles, KEYNOTE-054 protocol) — separate Regimen entity to capture the KEYNOTE-671-specific cycle count and NSCLC-specific monitoring (post-resection pneumonitis baseline elevated; lower irAE-discontinuation threshold). Excludes patients with EGFR-activating mutations or ALK fusion (FDA label) — driver-positive resectable NSCLC has superior adjuvant TKI options (osimertinib per ADAURA, alectinib per ALINA).
