MCL: alternating R-CHOP/R-DHAP induction followed by an ASCT pathway (MCL Younger-based)

Deterministic view of the source YAML entity. Clinical authority remains with the cited source IDs and reviewer sign-off state.

Knowledge-base record for audit. Provisional clinical content: this is not a prescription. An oncologist must verify original sources, applicability and review status. Full rules and constraints are in the YAML.

Notes

MCL Younger-based alternating induction: R-CHOP cycles 1, 3, 5 and R-DHAP cycles 2, 4, 6. Cisplatin is an essential R-DHAP component and was missing from the prior data. This is not the Nordic MCL2 maxi-CHOP/high-dose cytarabine regimen or a LyMA-style induction. Transplant conditioning and subsequent maintenance are not specified here; alternative conditioning must not inherit trial outcomes automatically. This record does not establish current first-line preference or individualized eligibility.

Evidence for specific statements

These citations cover only the statements and scopes shown. They do not establish clinical approval of this record.

In eviQ MCL R-DHAP, cisplatin is 100 mg/m² by IV infusion over 24 hours on day 1; cytarabine is 2 g/m² per dose by IV infusion over 3 hours twice on day 2, 12 hours apart. R-DHAP occupies cycles 2, 4 and 6, alternating with R-CHOP cycles 1, 3 and 5 at 21-day intervals. These are reference-protocol doses, not individualized dosing permission.

Induction component identity and timing only; not renal dose selection, current treatment preference or whole-record approval.

eviQ 1884 v6 and 1883 v6 treatment schedules.

The alternating R-CHOP/R-DHAP induction is distinct from Nordic MCL2 and LyMA induction. MCL Younger used a specific conditioning regimen; substituting BEAM or another conditioning regimen introduces uncertainty and does not authorize automatic transfer of trial outcomes.

Protocol identity and conditioning uncertainty only; not transplant prescription, maintenance schedule or efficacy estimate.
