Notes

Lymphodepleting conditioning phase for ide-cel (idecabtagene vicleucel, REG-IDECEL-MM-3L). Standardized in KarMMa pivotal phase-2 trial (Munshi NEJM 2021): fludarabine 30 mg/m² + cyclophosphamide 300 mg/m² IV daily × 3 days, days -5 to -3. Distinct from the 500 mg/m² Cy regimen used with axi-cel / brexu-cel — ide-cel pivotal trial used 300 mg/m². Single course immediately preceding CAR-T infusion; no maintenance. Coordinated administration before the CAR-T centre receives the patient. STUB — pending Co-Lead sign-off.
