Cetuximab rechallenge (mCRC RAS/BRAF-WT, post-1L EGFRi response, ctDNA-guided 3L+)

Deterministic view of the source YAML entity. Clinical authority remains with the cited source IDs and reviewer sign-off state.

Knowledge-base record for audit. Provisional clinical content: this is not a prescription. An oncologist must verify original sources, applicability and review status. Full rules and constraints are in the YAML.

Notes

CRICKET (Cremolini 2019, JAMA Oncol) and CHRONOS (Sartore-Bianchi 2022, Nat Med) established cetuximab rechallenge as a 3L+ option in mCRC patients who previously responded to a 1L EGFRi-containing regimen (FOLFOX-cetuximab or FOLFIRI-cetuximab), then progressed, then received an EGFRi-free 2L. ctDNA-guided selection (RAS-WT clearance on liquid biopsy at time of rechallenge) substantially improves response rate (CHRONOS: ORR ~30% in ctDNA-RAS-WT vs ~0% in mutant). Pre-rechallenge ctDNA RAS/BRAF testing strongly recommended.
