Carfilzomib + Rituximab + Dexamethasone (CaRD) × 6 cycles — for CXCR4-WHIM-mutated WM (BTKi-resistant subset)
Deterministic view of the source YAML entity. Clinical authority remains with the cited source IDs and reviewer sign-off state.
Knowledge-base record for audit. Provisional clinical content: this is not a prescription. An oncologist must verify original sources, applicability and review status. Full rules and constraints are in the YAML.

Notes
CaRD trial (Treon 2014): r/r WM — major response rate 87%, VGPR 68%; particularly active in CXCR4-WHIM-mutated subset where BTKi response weaker. Carfilzomib (irreversible proteasome inhibitor) has minimal peripheral neuropathy vs bortezomib — preferred for WM IgM-related neuropathy patients. Cardiac monitoring critical — carfilzomib has cardiotoxicity signal (CHF, hypertension) particularly in older patients. Weekly schedule less cardiotoxic but lower efficacy data in WM.
