Notes
IFCT-0501 / IFCT-0803 and meta-analyses (Quoix 2011 Lancet for carbo-paclitaxel weekly elderly schedule) established the principle of platinum-doublet benefit in fit elderly (≥70-75y) with attenuated dosing. For non-squamous histology, carbo-pem at AUC 4 + 400 mg/m² is a commonly-used reduced-dose schedule that preserves efficacy while lowering G≥3 cytopenia and fatigue in elderly. Add pembrolizumab (KEYNOTE-189 elderly subgroup, fit ≥75) when PD-L1 not low and no contraindication. Geriatric assessment (G8 / CGA) recommended before initiating chemo in this age group.
